North Santiam School District uses FinalForms for all athletic paperwork. Paperwork, physical, and payment must all be complete before your athlete steps on the field or mat.

1. District forms in FinalForms
Create or sign in to your North Santiam FinalForms parent account and complete every athletic form for your student: emergency contacts, insurance details, consents, and signatures.
2. Current sports physical
A physical must be on file and valid through the end of the season. Add the exam date to your parent portal so coaches can see your athlete is cleared.
3. Waiver, gear, and payment
Accept the liability waiver and gear policy during registration, then pay the $495 participation fee. Your spot is confirmed once payment is received.
